    When I say I'm doing pretty good, I mean I'm doing sort of fair, much better than poorly but not quite good.

    I think "pretty" in this context is a word that means, or used to mean, "sort of."

    It's middlin' 'tween "good" and "excellent", unless it is used sarcastically. In that case it means something between "awful" and "my kid could do better."
