How confrontative should pastors and church leaders be?
Within wisdom and within scripture.
Deepends on the situation. Give an example
I think most pastors do not wish to be confrontational. There are times though that it is necessary.
Toward believers, I think Hebrews 10:24 should apply. "And let us consider one another to provoke(literally, spur on)unto love and to good works."
I take that to be confrontational at times atleast. Toward unbelievers, I believe we need to study Jesus' example: How did He deal with unbelievers, can we not do likewise?
Not before or after church
It depends on what the pasor is being asked to confront.
