I have folders for each book of the bible.
Within each book-file I have the chapters.
I save interesting information I've collected in chapter-files associated with each book I'm studying.
I also have a separate personal note file associated with the book for my own study notes.

I'm not so organized when it comes to doctrine.
As I find interesting doctrinal notes I create a title based upon the doctrine.
It has become a bit unmanageable.

How do you organize your study notes?

I do similar to your description for Bible topics. For Doctrine I would probably create directories for the ten divisions of theology, then keep notes by sub topic with in. Or if you have Word or other word processor that will do table of contents, and keep things in one big document divided by the ten divisions and subdivisions.
