I would ask the church to consider that some may refuse to come, and would they be able to accept that.
But I suppose if the church voted to do so- I would oblige -

I would do what I always do and go with the majority. I’m not a fan of being forced to do anything neither should anyone else. But I’m just a sheep in the church.
I would vote no. I would leave if it came to that.
Last Sunday night pastor ask how many had Covid shot. Man on back row spoke up and said "only you and your wife have had it and the rest of us don't want it mentioned again."
I ride bicycles, between 100-150 miles a week, in 2020, 6200+ miles a lot of that on mountain roads. My bikes and smart trainer have power meters and I use a heart rate monitor. I may not know much about a lot of things in life but I do know what my normal heart rate for a given amount of power/weight effort and since getting the second shot the first week on Feb. (I'm a volly firefighter so I was first in line), since then my heart rate has gone up 10 bpm and my legs are complaining when they shouldn't be. When you are at a sustained (2-3 hours) HR of 165-170 bpm, 10 bpm is a lot. This summer I have seen my HR exceed 190 bpm which is an uncomfortable situation for this 63 year old.
Over the last 4 weeks I have been doing my best to power through this because I have a 65 mile charity ride end of the month (The Ben to the Shore Bicycle Tour - Ben Franklin Bridge in Philly to the Showboat in Atlantic City NJ). I'm a member of a large team (The Jim Thorpers). Normally I would expect to do this in less than 3.5 hours which is still my goal considering, but, I really need to get on it.

You need every bit of strength you can muster to use a halligan or K12 when you've already got bunker gear on.
The majority of my department has not gotten the vaccine. 2 members who did got COVID.
If the state tries to require it the department will probably just disband

Would leave a Church if they required a card as it's unbiblical.
James 2:1-3 is pretty clear on adding requirements, or showing favoritism to folks entering the building.
Not to mention that if a lost sheep is turned away from the Gospel at the service due to not having a vaccine card the Lord is going to hold the leadership accountable.
That’s terrifying. Have you considered getting your heart checked. I know younger folks can have heart problems as a side effect. Maybe your excellent condition has done the same.
