Someone goofed when they thought coins will replace paper!
Why can't they learn?
OH, I forgot.. they are our government! -
I used to work (35+ years ago) "rolling" new coins that came directly from the mint(s). Occasionally, there, I would see a 'blank' that got through the mint without being stamped in any way, plus an occasional "off-center" 'strike'. They were normally just sent on to the bank (although I put in a couple of 'good coins' for a couple of mis-strikes and still have them somewhere, I guess), which would, I presume, turn them back to the Fed. So I can tell you, it does happen.

The coins I used to roll, semi- manually, were at an average speed of some 3 to 6 mint bags per hour, for new coins. That's $1000 worth of dimes, quarters and/or halves per bag, for example, $200 in nickels, and $50 in pennies. (There were no dollar coins, in those days) The mint stamping machines do this at a much greater pace, and that all mechanically. Are you really surprised there is an occasional miss or mis-strike?
