Critical Incidents and Required Notification - Not Met
"The center failed to immediately notify the Parent of a significant event relating to the health, safety, or well-being of a child...C1 was bitten on her back a total of four times"

Child to Staff Ratio - Not Met
"the center failed to meet the required child to staff ratio"

Supervision Participation - Not Met
"both S2 and S3 failed to devote their time to supervising the (14) 2 year old children"

Report indicates that the toddler at the seminary preschool choked to death on a slice of smoked sausage!
What were they thinking?

Based on interviews, on 1/11/18, the center served...sliced smoke sausage cut into "quarter size" circular pieces, that are implicated in choking incidents.

failed to directly supervise 10 two year old children during lunch time

center staff failed to immediately notify emergency personnel of a critical incident involving a child in care...incident occurred at 11:16.00 am....contacted NOBTS Campus Police dispatch first at 11:26 am
